The Admissions Process

1. Physician determines needs for home care services

2. Hospital Discharge Planner, County Case Worker, Client or Family request AHC's services

3. Intake is completed and evaluation is set up

4. Clinical Manager and HR Generalist are assigned 

5. Care Plan is developed, preferences and house rules identified

6. Meet and Greets are set up

7. Training in home occurs, schedule is communicated

8. Care delivery begins
